As part of the Telecare Family, program will provide both behavioral health support services and case management services for 142 tenants living at Kearny Vista Permanent Supportive Housing (PSH). Services will be delivered onsite Monday through Friday 8am- 7pm, with provision for after hours on call response by the team. The program utilizes a Housing First service approach focused on voluntary engagement into case management and behavioral health services, housing stabilization and successful tenancy and community integration. Services are delivered through the provision of light case management and clinical case management services to effectively address the complex needs of tenants living at Kearny Vista PSH.

Full Time 1.0; AM 8:00 am - 4:30 pm; Monday - Friday
Wage range $15.00 - $19.74 Hourly – Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
POSITION SUMMARY
The Driver is responsible for transporting of members served to off-site appointments, as well as other transportation assignments.
ESSENTIAL FUNCTIONS
• Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders
• Coordinates and schedules members served transportation, as directed
• Transports members served to scheduled appointments and outings
• Documents members served appointments, behavior, and any pertinent interactions in the medical record
• Monitors members served during transportation
• Conducts vehicle safety check before each use
• When assigned to a nursing station, performs the duties of a Mental Health Worker, as needed
• Demonstrates knowledge of CPI
• If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
• If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
Duties and responsibilities may be added, deleted and/or changed at the discretion of management.
QUALIFICATIONS
Required:
• A High School diploma or a G.E.D
• Demonstrated sensitivity to behavioral health issues and multi-cultural populations is required (e.g., family member, volunteer, or previous experience, related classes, stated interest)
• One (1) year of professional driving experience is required
• A valid Class "C" California Driver’s License and insurability are required
• Acceptable driving record
• Must attain valid Class ‘B’ licensure (If transporting more than seven (7) persons served)
• Must be at least 18 years of age
• Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
• All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver’s license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. SKILLS
• Able to work independently with limited supervision

PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to stand, twist, squat, kneel and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend, and do simple and firm grasping of up to 50 pounds. The position requires manual deviation, repetition and dexterity and constantly drive.
